- Medications - Bring list of medications currently taking or
bring the medications with you.
- Referral Source - Name of physician, family member, or
friend that referred you.
- PCP - The date of last visit to a doctor for any reason,
cold, work exam, etc.
- If you cannot complete paperwork, please pick it up early to
fill out before your visit or bring someone with you to your
appointment who can help you.
